Click the START button, SETTINGS, CONTROL PANEL

Locate the MODEM icon and double click on it

Note the name of your modem, This is the Systems Name for your modem.
Click the Diagnostics tab.

Select the COM PORT your modem is on, Click the More Info button.

Note the line ATI3, this is the hardware name for your modem.

Click OK, CLOSE, close Control Panel.

If the System name and the Hardware name are differnt please send both names.
Also include the manufacture and model of your computer if it is a national brand name company like DELL, Gateway, Compaq, ect...
